MAP PROGRAM

The Membership Assistance Program(MAP) is a plan benefit available to all eligible plan participants of the Laborers' District Council of Western Pennsylvania Welfare Fund.  The Fund recognizes that personal concerns both on and off the job can affect a worker's health and job performance.  Left untreated, these problems can not only impact work performance, but may lead to higher accident rates, workers' compensation costs, and health care costs.  Some of the problems MAP addresses are: substance abuse, family problems, financial/legal issues, and mental health.  MAP seeks to help members by providing the following services:

IMPORTANT NOTICE ABOUT COORDINATION OF BENEFITS

EACH YEAR ALL MEMBERS MUST CONTACT HIGHMARKS DEDICATED LABORERS' CUSTOMER SERVICE UNIT TO PROVIDE INSURANCE INFORMATION. THIS MUST BE DONE EVEN IF YOU HAVE NO OTHER INSURANCE COVERAGE.

Failure to contact Highmark may result in claims not being processed timely.

Each calendar year, the initial claim for each member of your family will be pended until Highmark is contacted and told if you have other insurance coverage or not.

If Highmark does not receive the needed insurance information, the initial claim and all following claims will be denied until the information is received.

arrow Updating your insurance information is easy. All you have to do is call the member service number on the back of your Highmark ID card (1-866-594-1732) and confirm if you do or do not have other insurance coverage.

UNEMPLOYMENT BENEFITS INFORMATION

If you need to file for unemployment benefits, there are two ways to do it. One way is to file by phone. The phone system has been upgraded throughout to provide more efficient handling of claims. Another easier and faster way is to file online. This is the recommended way if you have access to a computer. Both the phone number and website are listed below.

The Department of Labor and Industry has also developed the tip card below to provide accurate, helpful information for individuals who are currently unemployed, or at risk of becoming unemployed. Click on the link below to view the card, and print one out to keep with you if you'd like.
